Responsible for performing case management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum. Performs duties telephonically. Primary duties may include, but are not limited to:
Ensures member access to services appropriate to their health needs.
Use appropriate screening criteria, knowledge, and clinical judgment to assess member needs.
Conducts assessments to identify individual needs and a specific care management plan to address objectives and goals as identified during assessment. Implements care plan by facilitating authorizations/referrals as appropriate within benefits structure or through extra-contractual arrangements.
Coordinates internal and external resources to meet identified needs.
Monitors and evaluates effectiveness of the care management plan and modifies as necessary.
Arkansas Residency is required.
Requires Master’s degree in social work, counseling, nursing or related behavioral health field. (BS or higher is required for RN.)
Current, active, unrestricted RN, LCSW, LPC, LMFT, LMHC license in state of AR is required.
Your experience must include a minimum of 3 years' clinical experience in social work, counseling with a broad range of experience with complex psychiatric and substance abuse, or substance abuse disorder treatment; or any combination of education and experience, which would provide an equivalent background.
Experience working with adults with substance abuse disorders and serious mental illness is required.
Experience with community mental health resources, CSB (Community Service Board), or similar is preferred.
Prior telephonic Case Management experience is preferred.
You must be competent in Microsoft Word, Excel, and Outlook.
