Responsible for providing consultative support to the largest accounts concerning all care programs to include care management, disease management, wellness, network management and other programs.
Primary duties may include, but are not limited to:
Provides support to the sales process in presentation of these programs and advises accounts on programmatic changes needed based on data collected.
Develops relationships with regional care management groups and works with them to improve operational interfaces.
Serves as a subject matter expert and trainer on care management operational issues.
Requires a BS in nursing or other related clinical area and minimum of 6 years related experience including program development, marketing, and clinical practice; or any combination of education and experience, which would provide an equivalent background.
Lean or Six Sigma
Experience with community health or based organizations
Health equity or health improvement project experience
Public Health or Healthcare Administration experience
